As a licensed childcare provider, the State of Colorado requires up-to-date immunization records or exemptions for all participants at Game On! Boulder. Below are the blank official State of Colorado forms, as well as a link to the CIIS Public Portal where you can find and download your camper's immunization records.

To upload your immunization record or vaccine exemption form to your Jumbula account after registration:

  • Go to "Enrollments" at the top of the page, then "Order History"
  • Click the arrow to the left of your camper's enrollment to expand it
  • Click the three dots in the Actions column and select "Follow-Up Forms"
  • Under Immunization/Exemption Forms, click "Fill it Now" (if it says View/Edit you have already uploaded your form)
  • On the next page, click "Edit"
  • On the next page, click "Choose File," then select your document
  • Once your document is uploaded, click "Submit"

Campers 10 and older are welcome to walk or bike home from camp alone. If you would like to authorize your camper to do so, you will need submit the Camper Walk Home Consent form to Game On!. You submit this form electronically by clicking here or you can download the form, fill it out and sign it, and either bring it on your camper's first day, or email it to [email protected].

Please note: campers under the age of 10 may walk home with a camper over 10, so long as both have walk home consent forms on file, and the arrangement has been communicated to Game On!.

The Parent/Guardian Handbook details our policies, procedures, and general information that you and your camper will need for a successful camp season. The handbook is linked in registration, and a signed agreement that you have reviewed it's information, policies, and procedures is required at the time of registration.
